China Medical Visa Options 2026: Visa-Free, Transit & S2 Compared
China now offers four realistic entry routes for medical travelers — 30-day visa-free entry for 86 nationalities, the 240-hour transit exemption, S2 medical visas, and pilot-zone arrangements. Picking the right one depends on treatment length; this guide compares them.

First, the headline: medical travel is visa-eligible
The single most useful fact of 2026: medical treatment is an officially recognized purpose of visa-free entry. Unilateral and mutual visa exemptions now cover around 86 countries by early 2026 — the UK and Canada joined in February, Kyrgyzstan and Vietnam in August — and foreign visitors entering visa-free for treatment are a documented, growing cohort. For most patients from Europe, Southeast Asia, Central Asia and the Gulf, the visa question is now "which exemption fits," not "how do I apply."
The four routes compared
### 1. Standard visa-free entry (30 days) — the default
- Who: passport holders of ~86 countries with mutual or unilateral exemptions.
- Fits: consultations, screening, dental work, TCM programs, minor procedures — anything completing within the exempted stay (typically 30 days).
- Watch: stay limits are fixed; extensions mean a visa run or an upgrade to S2. Hainan's 30-day policy covers 61+ nationalities and pairs with the Lecheng pilot zone.
### 2. 240-hour transit visa-free — the surgical-window hack
- Who: 55–57 eligible nationalities transiting to a third country through 65 ports.
- Fits: the "procedure inside a tourism trip" pattern — implants (first phase), health checkups, eye work, minor surgery. A documented British patient completed a full dental pathway in 13 days inside a transit window.
- Watch: you must hold an onward ticket to a third country and stay within permitted areas; 240 hours (10 days) is enough for many procedures but not for multi-phase orthopedics or a 6-week CAR-T protocol.
### 3. S2 medical-treatment visa — for courses longer than a month
- Who: anyone whose treatment plan exceeds visa-free windows, plus accompanying family.
- Fits: major surgery with recovery, rehabilitation programs, multi-visit treatment courses. The S2 is issued for "private affairs including medical treatment" — hospital invitation letters materially strengthen applications.
- Watch: apply at your nearest Chinese consulate; processing runs 1–2 weeks typically. Full mechanics in our dedicated S2 guide.
### 4. Pilot-zone and special arrangements
- Hainan: visa-free 30 days for 61+ nationalities plus the Lecheng new-drug channel — the route for therapies available only in the pilot zone.
- Proposed medical visa: legislators have proposed a dedicated medical-treatment category with certified-hospital invitation letters, treatment-length stays (1–2 years) and family accompaniment. Not yet law — but the direction of travel is clear.
The decision framework
Ask three questions in order:
- How long is the treatment plan in writing? Under 30 days → visa-free. Under 10 days and you can route through a third country → 240-hour transit. Longer → S2.
- Does your nationality appear on the exemption lists? If not, the tourist visa (L) route accepts medical-treatment itineraries, and the S2 is the cleaner label for hospital-based care.
- Is the therapy itself pilot-zone-only? If yes, Hainan's arrangements apply and often simplify everything else.
Documents that make every route smoother
Whichever entry route you use, carry:
- The hospital's admission confirmation or treatment plan (bilingual) — immigration officers increasingly expect it for medical-purpose entries.
- Proof of funds and return/onward tickets.
- Insurance documentation or a payment plan letter for expensive procedures.
- For S2: the hospital invitation letter, and medical records supporting the treatment need.
One caution worth repeating: never enter on a route whose activities don't match your actual purpose — a documented mismatch is the only common way medical travelers create immigration problems for themselves. China's door is open; walk through it honestly.
